1. Trim and pat dry the chicken breasts and season with salt and pepper.
2. Place onion, garlic, crushed tomatoes, chicken broth, and italian seasoning in the crockpot and stir to combine.
3. Lay the seasoned chicken on top of the tomato mixture, cover, and cook on LOW for 6–7 hours or HIGH for 3–4 hours.
4. Remove the chicken and shred with two forks, then return the shredded chicken to the crockpot.
5. Stir in the uncooked pasta or noodles and cook on HIGH for 20 minutes or until tender.
6. Pour in the heavy cream and parmesan cheese, stirring until the cheese melts and the soup is creamy.
7. Adjust salt, pepper, and herbs to taste.
8. Ladle into bowls and top with shredded mozzarella and fresh basil or parsley.
